What do I need to bring to the collection centre?

Please ALWAYS print your pathology request form when visiting the collection centre. The request form can only be used ONCE. You will also need to bring a valid photo ID (such as a Driver's license) for identification. You do not need to book an appointment.

How long are my tests valid for?

All tests are valid for 6 months from the date they were purchased. Each pathology request form can only be used one single time.
